Medicare rebate

If you have a chronic disease (incl. diabetes, high cholesterol, high blood pressure, cancer and/or stroke) or complex condition, you may qualify for Chronic Disease Management (CDM) services (eligibility is determined by your GP).  Under CDM services, Medicare rebates are available for a maximum of 5 Dietetic services per person per calendar year.

Medicare provides a rebate of $52.95 per consultation for those referred to a Dietitian by their GP under a CDM service.  Your GP needs to have provided a referral before your appointment for you to be eligible for this rebate.  Please bring a copy of your referral with you to your appointment to enable you to claim your rebate.  Melissa Selan does not bulk bill appointments. 

Private Health Insurance

Most private health funds provide rebates for a visit to the Dietitian. The rebate will depend on your level of cover.  Please contact your health care provider to check your entitlements. 

National Disability Insurance Scheme

Melissa works with those who are self-managed or managed through an Agency.  Services can be claimed under Improved Health & Wellbeing and Improved Daily Living and tailored to individual client on goals.

Melissa is familiar with the requirements and documentation needs of the NDIS and able to communicate with Support Co-ordinators and planners as needed.  The total Dietitian hours will be estimated on your face-face contact as well as what is done outside of the consultation such as time for nutrition analysis, establishing an individualised meal-plan, medical reports and liaising with other health providers. 

TAC

Melissa Selan is a TAC registered provider.  Melissa will invoice TAC directly so there are no out of pocket expenses to you.  Home visits are available if required.
